New Client Form

Please note we will contact you within 3 to 5 business days. Pauquette Center requires  new client paperwork to be signed and a copy of the client’s insurance card(s) before an appointment is scheduled or to be added to our waitlist. This form is for individual therapy.

  • If you are interested in a testing, please have your provider fill out our Psychological Testing Referral Forms.
  • If you are doing a referral for Comprehensive Community Services, please have your service facilitator fill out our CCS Pauquette Referral Form.
  • If you are interested in our DBT Comprehensive Program, please go to that service page and fill out the appropriate referral form.
