At Pauquette Center, we are committed to providing you with timely access to your health information while maintaining the highest standards of privacy. To request a copy of your records, please use one of the contact methods below to reach our Records Custodian, Tami Carlson. Please note: If you are requesting records for a disability claim, Wisconsin Disability (SSDB) will automatically request your records directly from us, and no further action is needed on your part. For all other requests, ensure you check your email after submitting our online form to provide the required electronic signature.
